Forms Required for Registration:

(Must be returned prior to the scheduled Parent Night. If not signed and returned by the second day of class, student will be dropped from the course)

This form is what gives us all the required information about the student such as their name, date of birth, and which class they would like to attend. It also outlines a few of the important terms and conditions for our program, and must be signed by both the parent/guardian and student to certify their understanding of these conditions.

On the top of the second page there is a FERPA Release of Information section. It is a program requirement that at least one parent/guardian be listed within this section if the student is under the age of 18. If a third party is paying for the students' tuition, it may be helpful to list their name as well.

The Parent Night registration is available at the bottom of the second page and must be complete if a parent/guardian would like to attend one of these free informational sessions. If no one wishes to attend, it may be left blank.

This form digs deep into what our program is and what our requirements of each student are. Every student enrolled in the Rogue Community College Driver Education program will be held to the requirements listed within this form. The first two pages are information only, but the last page must be signed by both the student and parent/guardian to certify your understanding and acceptance of our program requirements. 

Forms Required to Complete the Program:

(Must be signed and returned prior to the student receiving their Certificate of Completion, within the 90-day completion requirement)

It is a State requirement that, at some point during the course of this program, every student completes five (5) hours of supervised drive time in addition to the six (6) hours we provide with our instructors. This form is to be signed by the supervising driver, stating that the student has completed those hours.

While this document is not specific to the Driver Education program, every registered student has the same rights and responsibilities as listed in this document. The part we want you to pay particular attention to is Section 9, titled "Student Code of Conduct and Procedures" as all students are expected to adhere to the expectations outlined.
